Please arrive at least 15 minutes before the scheduled appointment so the required paperwork can be done before you see the Doctor. You can also download new patient forms by clicking HERE and fill them and bring it to your visit. You can also request eFORMS via text and fill them quickly. Just TEXT us to request new pt forms via text
Please bring your current Insurance card and a valid Photo ID such as a drivers license or a state issued photo ID. We will not be able to see you otherwise.
Please bring all your prescription medication bottles for this visit. This helps us document accurately what you are taking.
If you are transferring care from another provider, you will be required to fill a release of medical records form which will be sent to your provider for obtaining your medical records. Please have your previous providers' name and phone number with you.
PLEASE NOTE WE DO NOT PRESCRIBE NARCOTICS (OXYCODONE, OXYCONTIN, MORPHINE ETC) AND ANXIETY MEDICATIONS (XANAX, ATIVAN, KLONOPIN ETC) ON A CHRONIC BASIS. IF YOU ARE TAKING SUCH MEDICATIONS, PLEASE BE ADVISED WE WILL NOT BE ABLE REFILL THESE MEDICATIONS. WE WILL STILL SEE YOU FOR ALL YOUR OTHER NEEDS.
